Format:
This assignment is to be delivered in the format of a journal/diary. You are to explore the emotional path that an individual takes when newly diagnosed with Cancer and the diagnostic procedures, and treatments involved in their journey.
Length: maximum 2 pages (plus title page) with 2 drug cards attached.
Marking Criteria:
The marking criteria listed below outlines the expected content that is to be covered in this assignment. This is an individual student assignment. Each student is to hand in the assignment at the beginning of class on the due date. Late submissions will be deducted 10% per day to a maximum of 7 days at which time a 0 grade will be given. (Note: this is not an essay assignment and marks will be deducted for not following the assignment in a journal format). Each student is required to submit a marking rubric sheet with their Why Me Assignment at the time of submission. 10% will be deducted if the marking sheet is not submitted on time!!

Why Me? – Marking Rubric
/3 Title Page/Assignment Format (Student Name, Professor Name, Title, Due Date, Running Header, and Page Numbers-APA format)
/3 Spelling/Grammar (Under 2 errors)
/20 Medication Research-2 chemotherapy medications researched (10 marks designated for each medication research). Should be in the format of drug cards as outlined in previous PHAR 1000 drug card assignments). Please attach the drug cards to the paper assignment.
Students need to cover the following areas of content for each medication (note: drug card medications are required to be researched as commonly ordered treatment for the type of cancer that was identified in the student journal entries):

/2 In-text Citations (2 in-text citations, using the most current APA format)
/2 Reference Page-List of References used (1 text and 1 website references and they must be sited in the journal with in text citations)-following most current APA format). Note: cutting and pasting content will result in a 0 grade for the medication section.
