Health Disparities are a sad reality in many of our communities. For this assignment identify a disparity in a community within NYC (preferably) but can be in NY State, address the determinants of health that relate to this disparity and plan an intervention to target this disparity. Visiting the web site of your local or state (NY) Department of Health should provide data and examples of disparities they are targeting. Examples that are included obesity, diabetes, asthma, infant mortality and heart disease. You may choose one of these areas of concern or any other area with an acknowledged disparity. Step 2: As illustrated in the powerpoint, map the areas of your county, town or community that are experiencing a health disparity. Step 3: Examine the data available to analyze the root causes of the disparity. This might include poverty, health literacy, cultural beliefs and limited access to services among other social determinants of health. Step 4: Develop an Intervention strategy and describe how you would target the chosen community to more effectively address and reduce the disparity.
