Task:
Case study – you are a student nurse on your first clinical practicum and a new patient has arrived in your ward. Your NUM has told you to read the patient notes so you can familiarise yourself with the patient’s case. In particular the NUM wants you to identify any issues related to the social determinants of health that may impact the patient both on admission and discharge e.g. where the patient lives, who they live with etc.
In your case study response you will need to provide;
· a brief description of the patient background (this is found in the patient notes)
· a brief explanation of the social determinants of health and why they are significant to patient care and outcomes.
· an explanation of how issues arising from this case study relate to three social determinants of health – Food, Social Support and Stress
· 3 recommendations (1 for each SDH – Food, Social Support and Stress) to address these issues for this patient to enhance patient outcomes
